WELCOME TO THE HEALTH OFFICE
My name is Priscilla Chavez and I am the school nurse this year. Our school's health assistant is Patty Castro. We are excited to work with you and your child this school year.
APS Student Daily Questionnaire
Each day before sending your child to school, please review the following questions regarding your child’s health.

If you answer yes to any of the questions, please do not send your child to school. Contact a medical provider or call the COVID Hotline at 855-600-3453, then call the health office at your child’s school. You will also have to call in their absence to the front office so that their attendance can be coded appropiately.
Questions for APS Students Before They Go to School

Does your child currently have a temperature of over 100.4 degrees?

In the past 24 hours, has your child experienced:
-Temperature 100.4 degrees or higher, chills
-Sore throat, runny nose or congestion
-New frequent, dry cough (for students with chronic allergies/asthma, a change in baseline cough)
-Shortness of breath or difficulty breathing
-Diarrhea, vomiting or nausea
-Loss of taste or smell
-New onset of headache
-Fatigue or muscle or body aches
- Has your child recently been in close contact with anyone who has exhibited any of these symptoms?

Has your child recently been in contact with anyone who has tested positive for COVID-19?

Other reasons to keep your child home during illness
Any undiagnosed or unknown rash (a rash that has not been seen or treated by a health care provider).
If antibiotic treatment is needed, your child should remain home for the first full 24 hours of medication. Please call the health office if you have additional questions.
Remember if your childs' symptoms change, worsen or don’t get better please call your health care provider.

Pleaese notify the health office by phone or email if your child tests positive for covid, is a close contact to someone who tests positive or if someone in the household is positive for covid. We have a process in place at school and we are here to assist you in this process.

COVID vaccine clinic information
Below is a link to help you register for a COVID vaccine for you or your child if you are interested.
Register students for the covid-19 vaccine
Our students are beginning to get vaccinated. Once a student reaches 2 weeks after their second dose of covid vaccine they are considered fully vaccinated and will not be required to quarantine in the event that they are deemed to be a close contact to somebody with Covid-19 if they remain asymptomatic. Once your child has received both doses of vaccine, please email immunizations records to Patty Castro at patricia.Castro@aps.edu, the Health Assistant, and she will get them on file.
Chronic Health Conditions
Please let me know if your student has a chronic health condition for which s/he will need nursing support this school year.
If your student(s) will require medication or a procedure while at school, please have their provider fill out their specific form. See Health Forms. I cannot give a medication or carry out a procedure without this signed document. Your child's doctor/provider will have to complete these health forms every school year.

APS' Medicaid Department is available to help families enroll in Medicaid or refer for other eligible service. Please call 505-855-5261 or 505-855-9820 (Spanish speakers) for more information.
